Trinity Baptist Church (“TBC”) reserves the right to cancel or withdraw a registration from Vacation Bible School 2025 (VBS) based on improper behavior and conduct of the participant and/or based on the improper behavior and conduct of the participant’s parent(s)/legal guardian(s). In consideration for allowing participant to participate in Vacation Bible School 2025 and other activities at or sponsored TBC and in further consideration of TBC allowing me and participant to enter and use the facilities owned, leased or otherwise provided by the TBC (“Facilities”), on my own behalf and on behalf of any executors, heirs’ successors and assigns, and on behalf of the participant I acknowledge, appreciate, and agree to the following conditions: By completing this online registration, I represent that I am the legal guardian of the participant, have obtained permission from the parent(s) of the participant to enter into this agreement on their behalf, or am at least 18 years of age. I am aware of the inherent risk associated with participation in Vacation Bible School 2025 can result in injury to myself and/or participants. On behalf of myself and the participant(s),
out of the negligence of other participant(s) or a negligent act or omission of TBC or its agents and employees. By execution of this agreement, it is my intention to assume all risk of injury and hereby surrender and waive any rights to sue or exercise any legal right, including any right to expenses of litigation, court cost, and attorney’s fees.
WAIVER AND RELEASE: In consideration for allowing myself and/or participant to participate in Vacation Bible School 2025 and use the Facilities, I hereby release, waive, indemnify, forever discharge, agree to hold harmless and covenant not to sue TBC and/or its directors, trustees, officers, employees, agents, insurers, instructors, caregivers, and volunteers (collectively “its agents and employees”), from any and all claims, actions, causes of action, and damages of any kind whatsoever, including but not limited to claims for personal injury or property damage, arising out of participant(s) in any program or activity at TBC or the Facilities. This release of liability waiver form extends to all acts of negligence by TBC and is intended to be as broad and inclusive as permitted by the laws of the State of Texas. If any portion of this agreement is held invalid, it is agreed the balance shall, notwithstanding, continue in full legal force and effect. I acknowledge that participant’s participation in Vacation Bible School 2025 at TBC is strictly voluntary. I certify that I am over 18 (eighteen) years of age. I have carefully read the foregoing agreement not to sue and acknowledge that I understand and agree to all the above terms and conditions. Prior to completing registration for the participant, I have had an opportunity to ask any questions. I am aware that by registering the participant, I assume all risk and waive and release all substantial rights that I may have and possess.
INDEMNIFICATION: I agree to save and hold harmless TBC from and against any suits, actions, losses, damages, claims or liability of any character, type, or description including expenses of litigation, court cost, and attorney’s fees for injury or death to any person or participant(s), specifically including but not limited to the participant I have registered. I certify that I am over 18 (eighteen) years of age. I have carefully read the foregoing agreement not to sue and acknowledge that I understand and agree to all the above terms and conditions. Prior to this agreement, I have had an opportunity to ask any questions. I am aware that by entering this agreement, I assume all risk and waive and release all substantial rights that I may have and possess. EMERGENCY AUTHORIZATION: I give permission to medical personnel selected by TBC and/or its designee to order X-rays, routine tests, and treatment for participant. In the event of an emergency if neither I nor any of my emergency contacts can be reached, I hereby give permission to the physician selected by TBC to hospitalize, secure proper treatment, order injections and/or anesthesia and/or surgery to participant named above. I further authorize the release of medical information to appropriate medical personnel and/or health coverage insurance companies. I understand that if I do not have medical insurance, I, as the parent or guardian or the participant, will be responsible for any medical expenses in the event of a sickness and/or injury. I understand that there are risks involved in taking place in recreational activities and other activities related to participation in Vacation Bible School 2025.
PHOTO RELEASE: I hereby authorize Trinity Baptist Church ("TBC") to publish the photographs taken of my child for use in TBC printed publications, audio/visual media, and website. I acknowledge that since participation in publications, video, and websites produced by TBC is voluntary; we will receive no financial compensation. I further agree that my child's participation in any publication and website produced by TBC confers upon me/us no rights of ownership whatsoever. I release TBC, its contractors, and its employees from liability for any claims by me or any third party in connection with the registered individual's participation. *
